Case Managers assess each client's individual needs with the goal of maintaining their health and safety, allowing them to remain in their homes and communities rather than enter nursing home settings. With the clients, their families, and health care team, Case Managers develop a person-centered individualized care plan. The plan is monitored through monthly contact including in-home visits and adapting the service plan as needed.
