We offer a variety of different living arrangements for adolescents, adults, and elders with disabilities that range from part-time individual support services, shared living, and 24-hour supervised residential homes. These program models provide a continuum of services and accommodate a wide range of support needs including services designed to support people who may have multiple disabilities, complex medical and/or behavioral health needs, and whose needs are changing as a result of aging or other circumstances. We also design flexible residential supports that provide a bridge and formal transition to living with greater independence in the community.

Individual Supports - Our Individual Support Program provides flexible services that help people with intellectual, developmental and other disabilities successfully transition to and maintain independent living in the community. At CHS, we provide services using a team approach. Our managers, support staff, and clinicians work in partnership with each individual, their family members, and friends to fulfill each person's dream of living in a home of their own. Individuals are encouraged to make positive and healthy decisions that are consistent with the goals they establish. Services include: individual service plans; personal care and support; clinical support; outreach assistance; case coordination; 24-hour on-call response; counseling; advocacy; recreation and leisure support; home/living services.
