FHP-The Orange County Fatherhood program services 30 fathers with children 18 years and younger residing in Orange County. The project provides parenting classes and wrap around social services for fathers and families. In 2022, the project staff served over 100 fathers and provided wrap around services to include referrals for medical, employment and housing. A large percentage of the clients became employed with a living wage. Non custodial fathers increase child support payments and became more engaged in the lives of their children

The American Heart Associationwhose grant is intended to advance the development and implementation of a comprehensive resource development plan to expand organizational capacity by establishing a donor database, launching a signature fundraising event, and conducting targeted research on Social Determinants of Health (SDOH)related grant opportunities. cardiovascular health, reduce health disparities, and strengthen communitybased prevention efforts., to deepen its impact and sustainability. Concurrently, the initiative will deliver employment training and job placement services for individuals, focusing on the SDOH domains of economic stability and healthcare access and quality. By addressing these unmet needs, participants will increase their ability to earn a living wage, reduce poverty, and disrupt the cycle of health inequity and poor health outcomes.

CTG - Closing the Gap Program, the goal of Holden heights family health initiative is to improve their overall health outcomes of 400 Black and Hispanic residents in Holden Heights. The intervention has several components: outreach and screening, health assessment and health education classes and referrals to medical and socioeconomic services. In 2023 the project staff served 420 residents, providing screening for dental and primary care services, enrolled residents in GED and other technical programs. We measure health outcomes. Participants reduce BMI, increased intake of fruits and vegetables and increase exercise activities.
