THE HOMELESS OUTREACH PROGRAM IS DESIGNED TO CONTACT CLIENTS IN SHELTERS AND ON THE STREETS WITH THE GOAL TO TRANSITION INTO COMMUNITY-BASED HOUSING. IN 2025, OVER 200 INDIVIDUALS WERE SERVICED.IN THE CASE MANAGEMENT PROGRAM, CLIENTS MEET WITH A CASE MANAGER AT LEASE ONCE A MONTH TO WORK ON GOALS SUCH AS INDEPENDENT HOUSING, BENEFIT ACQUISITION, OBTAINING NEEDED CARE TO ADDRESS MENTAL/PHYSICAL HEALTH AND/OR SUBSTANCE ABUSE ISSUES.IN 2025, OVER 3,000 INDIVIDUALS WERE SERVICED .THE PEER ADVOCACY PROGRAM IS FOR INDIVIDUALS THAT REQUIRE NON-TYPICAL CASE MANAGEMENT. CLIENTS MEET WITH THEIR ADVOCATES AS OFTEN AS NEEDED.THESE CLIENTS FREQUENTLY FACE ACUTE ISSUES SUCH AS EVICTION,DOMESTIC VIOLENCE AND LOSS OF BENEFITS. IN 2025, CHOICE WAS ABLE TO SERVE OVER 300 INDIVIDUALS.OUR CRIMINAL JUSTICE PROGRAM INCLUDES CASE MANAGEMENT, PRETRIAL SERVICES THROUGHOUT WESTCHESTER COUNTY COURT SYSTEM,PEER ADVOCACY, AND A YOUNG ADULT DIVERSION PROGRAM. IN 2025, CHOICE ASSISTED OVER 1000 INDIVIDUALS.
