HOMETOWN HEALTH CENTERS PROVIDES ACCESS TO COMPASSIONATE, INDIVIDUALIZED HEALTH CARE AND WELLNESS RELATED EDUCATION FOR EVERYONE, REGARDLESS OF FINANCIAL, CULTURAL, OR SOCIAL BARRIERS. APPROXIMATELY 20,000 PATIENTS WERE CARED FOR IN HOMETOWN'S SERVICE AREA, INCLUDING SCHENECTADY AND MONTGOMERY COUNTIES. THE CENTER RECEIVED $3.5 MILLION IN SECTION 330 FEDERAL GRANT FUNDING IN 2022 TO SUPPORT SERVICES DELIVERED TO A LARGELY MEDICALLY UNDER-SERVED POPULATION. THE HEALTH CENTER PROVIDES A WIDE RANGE OF MEDICAL, DENTAL AND BEHAVIORAL HEALTH SERVICES. CLINICAL SERVICES ARE DELIVERED IN A PATIENT-CENTERED MEDICAL HOME MODEL OF CARE. THIS MODEL IS A WAY OF ORGANIZING PRIMARY CARE THAT EMPHASIZES COORDINATION AND COMMUNICATION. THE HEALTH CENTER PROVIDED SERVICES TO ROUGHLY 20,800 PATIENTS WITH ABOUT 65,800 VISITS IN 2025.
