The CHN Program served 289 clients and their families in five languages, including Vietnamese, Nepali, Korean, Burmese, and Chin Tedim. Our team of dedicated Community Health Workers (CHWs) assisted these individuals in navigating complex healthcare systems, securing essential resources, and improving their health navigation skills. Through our assistance, 95.37% of our Travis County clients have received or maintained health coverage. CHNs also assisted with 70 client applications for the Medical Access Program (MAP), 107 applications for the Supplemental Nutrition Assistance Program (SNAP), and provided transportation to 80 clients.
