During its fourteen years of operation, Transitions successfully moved 4,107 clients to permanent housing with 13,698 positive outcomes off the streets. In the organization's fourteenth year of operation, Transitions helped 3,527 unique clients in its various programs and services. The organization served 173,238 healthy meals and moved 322 clients into safe, permanent housing of their own while facilitating positive outcomes for 724 individuals. Furthermore, Transitions housed 74 veterans permanently. The approach of Transitions is to meet people where they are and reduce barriers to housing and services. The program offers a wide range of temporary housing and services. The Day Center, specialized beds, emergency beds, and program beds are key to outreach and services. It also performs outreach to help even more clients in the community. In addition, Monarch Manor opened in December with 11 studio apartments for individuals 62 and over.
