PROGRAM SERVES RESIDENTS WITH INTELLECTUAL DISABILITIES AND COEXISTING MEDICAL AND MENTAL HEALTH CONDITIONS IN BOTH A RESIDENTIAL AND COMMUNITY SETTINGS TO SUPPORT WELFARE AND SAFETY AND ASSIST THESE INDIVIDUALS IN LIVING THEIR BEST LIVES.DURING THE YEAR A TOTAL OF 9 RESIDENTS LIVED IN TWO HOMES AND TWO APARTMENTS. BESIDES HAVING A DIAGNOSIS OF INTELLECTUAL DISABILITY MANY HAVE MULTIPLE DIAGNOSES INCLUDING DOWN SYNDROME, AUTISM, DIABETES, DEMENTIA, KIDNEY DISEASE, EPILEPSY AND TRACHEOSTOMY. HRDHZIA IN ITS PARTNERSHIP WITH HOLY REDEEMER HEALTH SYSTEM IS UNIQUELY CAPABLE OF SERVING THE COMPLEX NEEDS OF THESE INDIVIDUALS IN THEIR HOME ENVIRONMENT. RESIDENTS ARE REGULARLY SUPPORTED IN EXPRESSING THEIR FAITH WITH FREQUENT ATTENDANCE AT RELIGIOUS SERVICES AS WELL AS FREQUENT INTERACTIONS WITH THE SISTERS OF THE REDEEMER AT THEIR PROVINCE CENTER AND REDEEMER HEALTH'S TRANSFORMATION CENTER.HRDZHIA ALSO PROVIDES COMMUNITY PARTICIPATION SUPPORTS TO THESE INDIVIDUALS. THIS IS A SERVICE UNDER THE OFFICE OF DEVELOPMENTAL PROGRAMS THAT EMPHASIZES THE ABILITY TO SUPPORT INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES IN ACTIVITIES THAT PROMOTE COMMUNITY INTEGRATION. PARTICIPANTS REGULARLY PARTICIPATING IN COMMUNITY-LED ACTIVITIES AT A LOCAL GYM, ART STUDIO, AND LOCAL MUSEUMS. THE NUMBER OF HOURS OF PROVIDING THIS SERVICE AVERAGED 15-20 HOURS PER WEEK PER INDIVIDUAL.
