Grant programs: The Rural Health Grant objectives were combined with two other non-profit organizations to support the initiatives of New York State's Rural Health Network Development Program prevention agenda of promoting healthy lifestyles and increasing quality and access to healthcare for all communities. Community and employee wellness programs include: chronic disease prevention/care management through diabetes and nutrition education classes for children and adults, healthy menu options in the organizations and school cafeterias, and walking and fitness programs in the community. Increasing access to/and quality of health care in the rural areas were accomplished with support for a rural medical and a physician assistant student, established Care Coordinators who performed pre-natal home care visits for at risk populations, and implemented processes and protocols for Patient Centered Medical Home projects.
