CFC IS THE CARE MANAGEMENT ORGANIZATION OR CMO SERVING THE YOUTH OF MIDDLESEX COUNTY, NJ. CFC'S COMMITMENT TO THE WRAPAROUND MODEL OF CARE COORDINATION ENSURES THAT YOUTH SERVED ARE PROVIDED AN EVIDENCE-BASED APPROACH TO MANAGING THEIR CARE. CFC SERVES YOUTH WITH NEEDS IN THE AREAS OF BEHAVIORAL HEALTH, SUBSTANCE USE, INTELLECTUAL AND DEVELOPMENTAL DISABILITIES, AND JUVENILE JUSTICE. CFC'S CARE MANAGERS FACILITATE "CHILD FAMILY TEAM" MEETINGS THAT INCLUDE ANYONE THE YOUTH AND/OR FAMILY IDENTIFIES AS THEIR SUPPORTS. THIS CAN INCLUDE AUNTS, UNCLES, CLERGY, A COACH, TEACHER, AND OTHERS; AS WELL AS MORE FORMAL SUPPORTS SUCH AS PSYCHIATRISTS, THERAPISTS; AND MENTORS TO DEVELOP A COMPREHENSIVE SERVICE PLAN. FORMAL SUPPORTS ARE VIEWED AS TEMPORARY SUPPORT WHILE THE TEAM FORMULATES A SUSTAINABLE PLAN FOR AFTER CFC'S WORK IS COMPLETED.

SPECIALIZED CARE COORDINATION FOR PROBLEMATIC SEXUAL BEHAVIOR COORDINATED FAMILY CARE RECEIVED FUNDS FROM THE NEW JERSEY DEPARTMENT OF CHILDREN AND FAMILIES TO PROVIDE SERVICES SPECIFIC TO YOUTH ENGAGING IN PROBLEMATIC SEXUAL BEHAVIORS (PSB). THE SPECIALIZED CARE COORDINATORS PROVIDE PSB-RELATED PSYCHOEDUCATION AND RESOURCES TO FAMILIES AFFECTED BY PSB, AS WELL AS REFER THOSE YOUTH AND FAMILIES TO TREATMENT SERVICES.
