Home Health & Hospice Care has been Southern New Hampshire's trusted nonprofit provider of home health, hospice, and palliative care for over 140 years. We deliver skilled, compassionate care for every stage of life-whether recovering at home, managing a serious illness, navigating end-of-life care, or seeking Bereavement Support to help cope with loss. With our dedicated 16 suite Community Hospice House, strong local roots, and a team of staff and volunteers who treat patients like family, we bring excellence, innovation, and guidance to the 25 communities we serve across southern New Hampshire. Generous support from the community allows us to fulfill our mission to care for patients who do not have adequate insurance or resources.
