CONCORDIA TELE CAREGIVERS

The mission of Concordia Tele CareGivers dba Concordia Private Care (CPC) is to serve our aging community with a continuum of high-quality caregiving options, provided in a Christian environment, and to serve those with limited funds to the best of our ability. Approximately 399 consumers were served across five counties in Western Pennsylvania. A total of 12,471 visits, equating to 15,196 hours of care, were delivered by our staff. We serve those over 65 in the community so that they may age in place in their home as safely as possible. Our staff of caregivers helps their consumers with functions of everyday living including, but not limited to: bathing, grooming, toileting, light housekeeping, laundry, grocery shopping, and meal preparation. Our consumers are referred to us from a wide variety of sources. These individuals are often nursing home eligible for their level of care. Our staff of caregivers provides Personal Assistance Services to this group of consumers. Our organization also accepts referrals from the VA to assist veterans who have been approved for in-home services including home support and personal care. Our administrative staff provides education to the community by speaking at different engagements on topics regarding senior independence. The CPC Private Duty Manager and the CPC Service Coordinators have addressed different consumer groups located at Concordia Continuing Care Retirement Communities within Butler and Allegheny County. Our goal is to educate the community we serve with the tools available to help them stay in their homes as safely as possible for as long as possible. We have continued to offer and arrange contracted spiritual visits for our consumers that were provided by a related organization as defined in Schedule R of this Federal Form 990 at no added cost. Total sponsorships during the fiscal year were $2,098. Employees of CPC were given the opportunity to participate in several mission events organized by Concordia Care Network dba Concordia Community Support Services. These are part of an ongoing Mission Committee that was formed to organize opportunities for employees to serve the community. The Mission Events during the fiscal year were: Back-to-School Collection Drive to support student care closet needs within the Butler, Freeport, and Knoch School Districts: July and August 2024. St. Luke Lutheran Church Cabot/Crossroads Mission/Central Westmoreland Habitat for Humanity Servant Day and House Build: Sept 2024, approx. 205 volunteer hours given by Concordia team over the two days. Toys for Tots Collection Drive for the chapters in Allegheny, Beaver, Butler, Greene, and Washington counties: Nov 2024. City Rescue Mission Collection Drive: April 2025, donated 200 hygiene items to support shelter needs. The Lighthouse Foundation Collection Drive to support Lighthouse transitional housing needs: Apr 2025, 50 laundry supplies items were donated. These sponsorships and Mission Events are some of the many ways we serve God by providing for the needs of His people.
