HAVE A HEART CONDUCTS FREE CARDIOVASCULAR SCREENINGS IN THE UNDERSERVED COMMUNITIES AND REFERS PATIENTS WITH INTERMEDIATE TO HIGH RISK TO THE CLINIC FOR CARE. THESE PATIENTS ALONG WITH OTHER CLINIC PATIENTS AT INTERMEDIATE TO HIGH RISK ARE ALSO REFERRED TO THE HAVE A HEART HEALTH EQUITY CENTER. THE NURSE EDUCATOR IN THE HEALTH EQUITY CENTER WORKS WITH THESE PATIENTS TO IMPROVE THEIR OVERALL QUALITY OF LIFE. THEY ASSESS THE SOCIAL DETERMINANTS OF HEALTH AND DEVELOP A PLAN WITH THE PATIENT TO IMPROVE THEIR OVERALL HEALTH AND WELL-BEING. REFERRALS ARE MADE TO OTHER COMMUNITY RESOURCES AS NEEDED. PATIENT DATA IS COLLECTED AND TRACKED TO MEASURE OVERALL IMPROVEMENTS.
