Grace Medical Home plays a crucial role in Central Florida's healthcare safety net by offering exceptional medical services to meet the holistic needs of low-income, uninsured individuals in the community. The Organization's patients include uninsured adults and children in Orange County who live at or below 225 percent of the Federal Poverty Level (FPL). Once enrolled, patients gain access to a comprehensive range of services, including primary and preventive care, in-house labs, digital x-ray, vaccinations, diabetes education, donated medications, and treatment for both acute and chronic illnesses. Additionally, the Organization provides on-site medical specialties such as cardiology, mental health counseling, dental care, endocrinology, ophthalmology, orthopedics, and more. What truly sets Grace Medical Home apart is the Organization's recognition at the local, state, and national levels for delivering this unparalleled level of care. The Organization's commitment to serving the underserved with comprehensive and compassionate healthcare has earned the Organization a distinguished reputation, making it a vital resource in the community.In 2024, the Organization transitioned to the Electronic Medical Record System after 13 years on the prior system. With this massive change, the Organization still served 1,258 unduplicated patients. These patients had more than 16,099 patient encounters, received more than $13,157,000 in value of care, including prescription medications obtained on behalf of specific patients or donated directly to the Organization's facility, and were offered 35 medical specialties on-site. The Organization's Mobile Medical Unit continues to go to underserved neighborhoods and had 338 patient visits on the unit, eliminating the transportation barrier that exists for 221 patients. Thanks to the 541 volunteers and staff, the Organization provided nearly 2,000 mental health visits, 1,775 dental visits, 651 social service visits, provided more than 600 spiritual care encounters, and distributed 2,125 food boxes to food-insecure patients.
