MARYS HOME CARE, A SPECIAL NEEDS CERTIFIED HOME HEALTH AGENCY (CHHA), WAS ESTABLISHED IN 2010 TO EXPAND UPON A LONG-STANDING COMMITMENT TO PROVIDING CARE FOR CHILDREN WITH SPECIAL NEEDS IN A CARE DELIVERY SETTING WHERE CHILDREN CAN HEAL AND THRIVE. TODAY, WE REMAIN THE LARGEST AND MOST COMPREHENSIVE PROVIDER OF SERVICES TO CHILDREN WITH SPECIAL HEALTHCARE NEEDS, DELIVERING CARE INTO THE HOMES OF THOUSANDS OF PATIENTS WHO REQUIRE SPECIALIZED MEDICAL, EDUCATIONAL AND REHABILITATIVE CARE. ST. MARYS HOME CARE PROVIDES SHORT-TERM SKILLED CARE FOR PATIENTS FROM NEWBORN TO 44 YEARS OF AGE. ST. MARYS HOME CARE, IS THE SOURCE FAMILIES TRUSTAND WITH GOOD REASON: WE PIONEERED THE FIRST LONG-TERM PEDIATRIC HOME HEALTHCARE PROGRAM IN NEW YORK OVER 30 YEARS AGO, AND REMAINS THE LARGEST AND MOST COMPREHENSIVE PROVIDER OF SERVICES TO CHILDREN WITH SPECIAL HEALTHCARE NEEDS. THE CHILDREN AND YOUNG ADULTS SERVED BY ST. MARYS HOME CARE ARE OFTEN MEDICALLY FRAGILE AND WITHOUT HOME CARE SERVICES, WOULD OTHERWISE REQUIRE HOSPITALIZATION OR INSTITUTIONAL CARE. THEIR DIAGNOSES RUN THE GAMUT OF CHILDHOOD SEVERE ILLNESSES INCLUDING COMPLICATION OF PREMATURITY, GENETIC DISORDERS, NEURO-DEVELOPMENTAL DISORDERS, NEURO-MUSCULAR DISORDERS, TRAUMATIC BRAIN INJURY, AIDS, CANCER, RESPIRATORY DISORDERS, CARDIAC DISORDERS AND VENTILATOR DEPENDENCE. ST. MARYS HOME CARES USES A FAMILY-CENTERED, TEAM APPROACH TO CARE. OUR GOAL IS TO PROVIDE EXTRAORDINARY CARE AND SUPPORT SERVICES IN THE HOME TO HELP PATIENTS REGAIN INDEPENDENCE. SERVICES PROVIDED BY ST. MARYS HOME CARE INCLUDE COMBINATIONS OF PROFESSIONAL HEALTHCARE SERVICES AND PERSONAL CARE SERVICES THAT ARE TAILORED TO MEET THE NEEDS OF BOTH PATIENTS AND THEIR FAMILIES. SERVICES MAY INCLUDE NURSING CARE, REHABILITATION THERAPIES, PERSONAL CARE BY HOME HEALTH AIDES, PAIN MANAGEMENT, DISEASE AND MEDICATION EDUCATION AND MANAGEMENT, TELEHEALTH, ARRANGING FOR EQUIPMENT, AND COORDINATION OF CARE WITH THE PATIENTS PHYSICIAN, FAMILY AND OTHER HEALTH CARE PROFESSIONALS DURING THE COURSE OF TREATMENT.
