National Center for Complex Health and Social Needs-The National Center for Complex Health and Social Needs aims to improve the outcomes for people with complex medical, behavioral, and social needs. It works to coalesce a new field of care by bringing together a broad range of clinicians, researchers, policymakers, and consumers who are developing, testing, and scaling new models of team-based, integrated care. The Center and its staff collaborate with other experts across the nation to advance best practices, inform policy and research, and foster an engaged community to develop this work and teach it to others.

Care Management Redesign Initiatives-The RHH aims to deliver more coordinated, efficient, and effective care to residents of South Jersey, which includes many of the region's most vulnerable citizens. The Camden Coalition RHH, along with RHHS' in Trenton, Newark, and Passaic, are bringing together regional partners (Area hospitals, behavioral health providers, community residents, and organizations) to highlight and close gaps in our communities.

Data quality and improvement-The Coalition works to break down informational silos within the healthcare system and across sectors by sharing, analyzing, and evaluating quantitative and qualitative data. Our integrated data framework highlights ways to improve upon our interventions and the complex systems (e.g., medical, behavioral, and social services) with which our patients interact. The Coalition runs a healthcare information exchange and an integrated database that brings together data from health care, criminal justice, school district, etc. sources.
