IADHS IS THE ONLY LICENSED ADULT DAY HEALTH CARE PROVIDER IN THE CITY OF IRVINE THAT ADMITS AND IS ABLE TO ADDRESS THE CRITICAL NEEDS OF OUR MOST VULNERABLE AND ISOLATED OLDER AND DISABLED ADULTS. OUR PROGRAM AND COMMUNITY-BASED SERVICES SPECIALIZE IN CREATING A PROGRAM THAT IS PERSONALIZED TO THE UNIQUE NEEDS OF THE PARTICIPANT WHILE HONORING THEIR DESIRE TO LIVE INDEPENDENTLY IN THE COMMUNITY OR THEIR HOME. LAST FY WE PROVIDED 6929 DAYS OF CARE TO ENROLLED DISABLED AND OLDER ADULTS. OUR PARTICIPANTS ARE PREDOMINANTLY UNDERSERVED AND UNDERINSURED OLDER ADULTS MANAGING ALZHEIMER'S DISEASE AND OTHER CHRONIC HEALTH CONDITIONS. MANY ARE CONSIDERED FRAIL AND AT-RISK OF ISOLATION, DEPRESSION, HEALTH COMPLICATIONS, AND FURTHER DETERIORATION. WE PROVIDE A CRITICAL SERVICE IN THE COMMUNITY RECEIVING 205 INTAKES AND 41 TOURS.
