Utilized the State of Illinois messaging campaign to promote consistent and inclusive safe sleep messaging. Increased awareness about infant safe sleep practices and provide risk reduction education and training for parents, caregivers, and early childhood professionals. Provided trainings/education including , but not limited to, maternal stress, breastfeeding promotion, smoking cessation and sleep environments. Identify social determinants of health and other barriers to safe sleep practices amongst families in the community, and adopt culturally relevant resources/approaches to address these barriers (e.g., cribs, pack-n-plays, co-sleepers, sleep sacks).Employed risk reduction approaches that will acknowledge families existing practices and provide additional opportunities to improve sleep environment and other safe sleep practices.

The Carolyn Adams Ticket for the Cure Patient Navigation Grant (AWARE) A program administered by the Illinois Department of Public Health,we assist individuals in navigating the breast cancer care continuum. The grant is part of the broader Carolyn Adams Ticket for the Cure initiative, which funds breast cancer awareness, education, and support services in Illinois. Community health workers work closely with our local hospitals, clinics and cancer resource centers and our team will work with coordinating communications and building rapport amongst clients and healthcare systems. We utilize our partnerships with healthcare providers, clinics, hospitals, and community organizations to facilitate access to healthcare services, coordinate care, and address social determinants of health. We collaborate with faith-based organizations, community leaders, and grassroots initiatives to enhance outreach, build trust, and leverage existing networks for community engagement.
