SKILLED HOME CARE CLINICIANS, IN COLLABORATION WITH PHYSICIANS, OFFER SERVICES SUCH AS PROFESSIONAL NURSING, INFUSION THERAPY, INTERMITTENT CARE, MEDICAL SOCIAL SERVICES, HOME HEALTH AIDES, AND SPEECH THERAPIES. THROUGH VNH&HC, SERVICES PROVIDED ARE GUIDED BY THE HOSPICE PHILOSOPHY IN WHICH THE FOCUS OF CARE IS ON THE QUALITY OF AN INDIVIDUAL'S LIFE. THE GOAL IS TO IMPROVE THE PATIENT'S QUALITY OF LIFE THROUGH PAIN AND SYMPTOM MANAGEMENT, IMPROVE THE FUNCTIONAL CAPACITY FOR PATIENTS WITH SERIOUS CHRONIC AND TERMINAL ILLNESSES, AND MEET THE NEEDS OF THE PATIENT'S FAMILY MEMBERS.
