With the generous support of our core partners and funders, AVOC's model promotes resident driven services that are positively impacting health disparities, at the neighborhoods level. In 2018, AVOC, home of the Greater Cleveland Community Health Workers Association (GCCHWA), increased its capacity to serve as the community engagement catalyst for Cuyahoga County's community health initiatives housed under the HIP-Cuyahoga umbrella, known as REACH. AVOC not only provides support as the connector of the clinic to community linkages strategy but the core programs of AVOC braided with Asset Based Community Development (ABCD) to work collaboratively with the all functions of the initiative helping to develop and shape a comprehensive community engagement framework.
