THE ORGANIZATION PROVIDES HOME HEALTH AND HOSPICE CARE TO THE RESIDENTS OF KOSCIUSKO COUNTY. THROUGH THE DIRECTION OF THE PATIENT'S PHYSICIAN, THE ORGANIZATION PROVIDED PROFESSIONAL HEALTH SERVICES TO 17 PATIENTS CONSISTING OF 206 IN-HOME VISITS BY NURSES, THERAPISTS, HOME HEALTH AIDES, AND HOMEMAKERS, ALLOWING THE PATIENT TO STAY AT HOME WHILE THEY RECOVER FROM MEDICAL TREATMENT. THE ORGANIZATION ALSO PROVIDED END OF LIFE CARE TO 43 PATIENTS CONSISTING OF 1,676 DAYS OF SERVICE. SUCH SERVICES PROVIDED BY NURSES, SOCIAL WORKERS, AND HOME HEALTH AIDES FOCUS ON PAIN AND SYMPTON CONTROL, AND INCLUDE THE PROVISION OF MEDICATION, SUPPLIES AND MEDICAL EQUIPMENT AS WELL AS INSTRUCTION, SUPPORT, RESPITE SERVICES AND BEREAVEMENT CARE.
