Strengthening Communities, Supporting Independence: LAHN's 2024 Impact In 2024, the Living at Home Network (LAHN) continued to lead the way in transforming healthy aging across Minnesota. We supported a vibrant network of 32 nonprofit organizations, empowering them to serve more than 8,000 older adults across the state. At the heart of our model is the power of community-local volunteers who provide essential services like transportation, friendly visits, homemaking support, and meal delivery, helping older adults remain safely and comfortably in their homes.Our work this year focused on four key pillars: 1. Technical Assistance & Grant Support - We helped our member organizations access and manage public funding efficiently, ensuring critical services could continue and grow. 2. Advocacy & Representation - By amplifying the voices of older adults and our member organizations at local, regional, and state levels, we advanced policies in partnership with groups like the Minnesota Leadership Council on Aging. 3. Operational & Administrative Efficiencies - We streamlined backend operations, reduced costs, and leveraged collective buying power-allowing our members to focus more of their energy where it matters most: direct service. 4. Member Networking & Best Practices - Through knowledge-sharing and collaboration, we strengthened aging-in-place strategies across the state. Our September Member Conference brought together 100 professionals for breakout sessions, peer learning, and meaningful connection. Through every initiative, LAHN stays true to its mission: to transform the landscape of healthy aging across Minnesota by empowering our community-based partners with resources, advocacy, and collaboration.
