PARTNERSHIPS FOR PERMANENCE- from here on known as- P4P: 53 young people participated in 6 P4P programs or received information about community resources from P4P related to: housing, mental health, career, education, case management, driver's license training, support group etc.** 18 young people were actively participating in LCS programs/activities or received case management, 23 young people received service referrals, and 12 young people participated in personal and professional development trainings.** 10 young people participated in Hennepin County Court Hearing observations and internal meetings, and developed a greater ability to advocate for themselves. P4P and Hennepin County Youth Advisory Group (YAC) empowered young leaders to use their lived experience as a tool for meaningful change, equipping them with the skills, knowledge, and voice needed to influence systems and advocate for themselves and their peers. 9 young people completed five YAC training modules, learned the power of their lived experiences, developed skills to share their impact stories, and gained confidence in navigating adult spaces.** 15 young people received professional clothing during the 2nd Annual P4P and Ramsey County Youth Resource Fair Clothing Drive. Our P4P young leaders were involved from the initial planning stages through the execution of the event.Young people actively engaged in the fair to access housing, educational, and medical resources.Young people gained leadership and teamwork experience by contributing ideas and taking on leadership roles in planning the Youth Resource Fair.** 13 Young people attended the 2025 P4P Wellness Event, where they were empowered to rest and practice self-care. Activities included yoga, body movement exercises, and reflection activities, helping participants relax, reconnect with themselves, and build healthy coping skills. Young people participated in discussions and activities tailored to their needs. Participants gained practical strategies for managing stress and navigating challenges.** 11 Young people participated in monthly support groups that provided increased awareness and understanding of healthy relationships, emotional regulation and trauma-informed coping skills.
