THE FILING ORGANIZATION IS A PART OF THE ASCENSION CATHOLIC HEALTH MINISTRY. ASCENSION IS A FAITH-BASED HEALTHCARE ORGANIZATION THAT DELIVERS PERSONALIZED, COMPASSIONATE CARE TO ALL, ESPECIALLY TO THOSE WHO NEED IT THE MOST. VIA CHRISTI FOUNDATION FUNDS A VARIETY OF AREAS INCLUDING CHILDREN'S MIRACLE NETWORK HOSPITAL AT ASCENSION VIA CHRISTI HELPING CHILDREN THROUGHOUT KANSAS WITHOUT REGARD TO ILLNESS OR ABILITY TO PAY, HOSPITAL CAPITAL IMPROVEMENTS AND MANY PROGRAMS THAT IMPROVE THE OUTCOMES AND SERVICES PATIENTS RECEIVE DURING THEIR CARE. FOR EXAMPLE: THE VIA CHRISTI FOUNDATION SUPPORTS THE ASCENSION VIA CHRISTI TRANSITIONAL CARE CLINIC (TCC) WHOSE PURPOSE IS TO PROVIDE CARE FOR THOSE WITH MEDICAL ILLNESS AND SOCIAL DETERMINANTS OF HEALTH. DURING FY23 THE TRANSITIONAL CARE CLINIC SERVED OVER 1,623 UNIQUE PATIENTS WITHOUT INSURANCE, UNDERINSURED OR PENDING DISABILITY CLAIMS; 60% OF THESE WERE TRANSITIONED TO A PRIMARY CARE PHYSICIAN. TRANSITIONAL CARE ALSO PROVIDES A HEART FAILURE CLINIC THAT OFFERS CHRONIC DISEASE MANAGEMENT PROGRAMS FOR THOSE WHO LIVE WITH HEART FAILURE AND/OR COPD. THEY FOCUS ON EDUCATION AND SELF-MONITORING TO HELP REDUCE READMISSION AND INCREASE QUALITY OF LIFE. THE FOUNDATION ALSO SUPPORTS BEHAVIORAL HEALTH PATIENTS THROUGH SCHOLARSHIPS FOR THE PARTIAL DAY OUTPATIENT INTENSIVE THERAPY PROGRAM. REGARDING PATIENT ASSISTANCE, THE FOUNDATION UNDERWRITES TRANSPORTATION, MEDICAL DEVICES AND PHARMACY NEEDS. THROUGH CMN-RELATED FUNDING EFFORTS, WE HELP PROVIDE STATE-OF-THE-ART MEDICAL EQUIPMENT FOR THE PEDIATRIC, PEDIATRIC INTENSIVE CARE AND NEONATAL INTENSIVE CARE DEPARTMENTS AS WELL AS DIRECT FAMILY AND PATIENT SUPPORT FOR TRAVEL, MEDICAL DEVICES AND MEDICATION.
