Kochavim Behavioral Health, Inc. The Care Model will involve coordination with various Federally Qualified Health Centers ("FQHC") who will work collaboratively with the Organization to develop uniform standards of quality behavioral health care, whereby patients receive services that are tailored to their specific behavioral health issues, yet adhere to the highest levels of evidence based care on a consistent basis.
