HAS MULTIPLE FULL-SERVICE OUTPATIENT AND CLINIC PHARMACIES IN ASCENSION HEALTHCARE HOSPITALS AND CLINICS IN THE MILWAUKEE METROPOLITAN AREA. THE PHARMACY'S NOVEL HOSPITAL DISCHARGE PROGRAM ASSISTS PATIENTS BY FILLING THEIR DISCHARGE PRESCRIPTIONS BEFORE GOING HOME WITH BEDSIDE DELIVERY AND CONSULTATIONS BY THE PHARMACIST. THE SAME PHARMACIST MAKES FOLLOW-UP PHONE CALLS TO THE PATIENT AT HOME FOR QUESTIONS, MONITORING, OR ACCESS TO MEDICATIONS. PHARMACY STAFF WORKS DILIGENTLY TO HELP PATIENTS WITH SPECIAL NEEDS AND FINANCIAL CHALLENGES. THE PHARMACY PROVIDES TECHNICAL RESOURCES AND OVERSIGHT IN MANAGING A NETWORK OF NON-WHEATON FRANCISCAN HEALTHCARE CONTRACT "340B" PHARMACIES IN SOUTHEASTERN WISCONSIN, SUCH AS WALGREENS AND WALMART, WHO FILL PRESCRIPTIONS FOR OUR PATIENTS IN EMERGENCY SITUATIONS AND AFTER HOURS.
