THE BELLIN HEALTH FOUNDATION, INC. THE FOUNDATION ALSO OPERATES THE GIFT SHOP AT BELLIN MEMORIAL HOSPITAL, INC. A SIGNIFICANT PORTION OF FUNDS RAISED BY THE FOUNDATION ON BEHALF OF BELLIN HEALTH SYSTEMS, INC. AFFILIATES ARE TRANSFERRED TO AFFILIATES FOR USE AS DIRECTED BY DONORS. THE BELLIN HEALTH FOUNDATION'S VISION IS TO ENABLE EMPLIFY HEALTH BY BELLIN TO PROVIDE WORLD-CLASS CARE WITHOUT FINANCIAL COMPROMISES.
