THE POLK COUNTY DAC PROVIDES TRANSPORTATION TO MOST CONSUMERS ATTENDING THE DAC. THIS SERVICE INCLUDES TRANSPORTATION TO AND FROM HOME TO THE DAC. THE BUS/VAN WILL GO DIRECTLY TO A CLIENT'S HOME AND PICK THEM UP AT THE BEGINNING OF THE DAY AND BRING THEM HOME AT DAYS END. THE DAC PROVIDES A CURB TO CURB SERVICE; MEANING THE CLIENT IS ABLE TO (PERHAPS WITH ASSISTANCE) GET OUT OF THE HOUSE ALONE AND TO THE VAN. IF THEY REQUIRE THE USE OF A WHEELCHAIR LIFT, OUR DRIVER WILL OPERATE THAT AND PUT THE CLIENT ON THE LIFT. WE WILL ASSURE THE USE OF SEATBELTS WHETHER AMBULATORY OR IN A WHEELCHAIR.THE POLK COUNTY DAC ALSO PROVIDES TRANSPORTATION DURING THE DAY. IF THEY HAVE WORK SITES, WE PROVIDE TRANSPORTATION TO AND FROM THAT WORK SITE. IF THEY GO INTO THE COMMUNITY FOR EMPLOYMENT, IF DURING DAC HOURS, WE GENERALLY PROVIDE THAT SERVICE AS WELL. THERE ARE TIMES WHEN OTHER ARRANGEMENTS HAVE BEEN MADE DEPENDING ON THE TIME OF DAY. FOR EXAMPLE, WE HAVE HIRED TRI-VALLEY BUS TO PICK UP ONE PERSON AS THAT RIDE OCCURS OVER OUR LUNCH HOUR OR IN THE EVENING AND STAFF WAS UNAVAILABLE TO DO THAT TRANSPORTATION.TRANSPORTATION RATES VARY DEPENDING ON WHETHER THE CLIENT IS IN A WHEELCHAIR, USING A WHEELCHAIR, MILES DRIVEN AND COUNTY APPROVAL.
