PROVIDE CARE AND LODGING FOR INDIVIDUALS WITH DEVELOPMENTAL AND OTHER DISABILITIES IN PLEASANT, SAFE SUBURBAN NEIGHBORHOODS, INCLUDING THE INTERMEDIATE CARE FACILITY (ICF/MR) GROUP HOMES AT OUR VICTORIA CAMPUS AND SUPPORTED LIVING SERVICES (SLS) CARE IS PROVIDED IN FOUR-PERSON, SINGLE FAMILY COMMUNITY HOMES ALONG WITH MULTI-BED FACILITIES THROUGHOUT HENNEPIN, CARVER AND SCOTT COUNTIES. WE SERVE APPROXIMATELY 130 CLIENTS FOR WHICH WE PROVIDE 1) CARE AND SUPERVISION 24 HOURS PER DAY BY SKILLED AND CARING STAFF, 2) TRAINING IN ACTIVITIES OF DAILY LIVING, 3) THERAPEUTIC LEISURE AND RECREATION ACTIVITIES, 4) NURSING AND BEHAVIOR ANALYST SERVICES AS NEEDED AND 5) A PERSON CENTERED APPROACH IN ALL SERVICES PROVIDED.
