SERVED 6 INTELLECTUALLY AND PHYSICALLY DISABLED ADULTS AND CHILDREN IN THEIR FAMILY HOMES OR IN INDEPENDENT LIVING SITUATIONS. EACH IN HOME CLIENT IS SERVED EACH WEEK WITH VARYING NUMBER OF HOURS DEPENDENT UPON INDIVIDUALIZED NEEDS DETERMINED BY THE COUNTY CASE MANAGER. OBJECTIVE IN EACH TYPE OF CARE IS TO ALLOW EACH CLIENT TO BE ABLE TO LIVE IN THE LEAST RESTRICTIVE SETTING IN THE COMMUNITY.
