IT PROVIDES HOME CARE AND HOSPICE SERVICES IN THE COMMUNITY, IN ASSISTED LIVING HOUSING WITH SERVICES BUILDINGS, APARTMENT BUILDINGS, NURSING HOMES AND OTHER INDEPENDENT LIVING UNITS. OUR HOME CARE AGENCY HAS ITS CORPORATE OFFICE IN ELK RIVER, MINNESOTA, AND ONE BRANCH OFFICE IN CAMBRIDGE, MINNESOTA. OUR HOSPICE AGENCY IS LOCATED IN LOCATIONS; ELK RIVER, BUFFALO AND CAMBRIGE, MINNESOTA.WE ACCEPT PAYMENT FROM MEDICARE, PRIVATE PAY, MEDICAL ASSISTANCE, HEALTH PLANS, AND PRIVATE INSURANCE. OUR CLIENT BASE IS A MIX OF PRIVATE PAY, VA, MA & MEDICARE ELIGIBLE BENEFICIARIES.WE HAVE PROVIDED SERVICES TO CLIENTS THAT WE DID NOT CHARGE THEM FOR BECAUSE THEY COULD NOT AFFORD IT. WE OFFERED HOSPICE CARE TO A FAMILY WHOSE INSURANCE COVERAGE WILL LAPSE SOON AT A REDUCED RATE TO THE FAMILY. OUR MISSION IS TO SERVE ALL CLIENTS NO MATTER WHAT THEIR PAYMENT SOURCE OR BACKGROUND IS, IF WE CAN PROVIDE THAT SERVICE.ANY REVENUE EARNED IS TURNED BACK INTO OUR BUSINESS TO PROVIDE OTHER SERVICES TO OUR COMMUNITIES. WE HAVE ONE BIG EVENT IN JULY CALLED OUR BUTTERFLY RELEASE, TO HONOR THOSE PATIENTS WHO HAVE DIED IN OUR HOSPICE PROGRAM OVER THE PAST YEAR.ALL OF OUR DONATIONS ARE USED TO SUPPORT THE HOME CARE AND HOSPICE PROGRAM SUCH AS MASSAGE THERAPY, PET THERAPY, AND MUSIC THERAPY. ALL OF WHICH ARE SERVICES PROVIDED BUT NOT COVERED AS A BENEFIT TO HOSPICE PATIENTS. OUR CHAPLAIN PROGRAM IS ANOTHER SERVICE STARTED WITH DONATED MONEY FROM COMMUNITY MEMBERS OR FAMILIES.
