INDIVIDUALIZED SUPPORTED LIVING (ISL) - ISL HOMES ARE DESIGNED TO SERVE ONE TO FOUR INDIVIDUALS AND ARE LOCATED IN RESIDENTIAL, SINGLE-FAMILY NEIGHBORHOODS. A SUPERVISOR COORDINATES THE INDIVIDUALS CARE, STAFF SCHEDULES, AND COMMUNITY INTEGRATION AS WELL AS STAFF SUPPORT. INDIVIDUALS LIVING IN ISL HOMES ARE PROVIDED UP TO 24 HOURS PER DAY SUPPORT BASED UOPN THE NEEDS OF THE INDIVIDUAL(S). THE GOAL IS TO ENABLE THE INDIVIDUAL(S) TO LIVE SAFE, HEALTHY LIVES IN A CLEAN ENVIRONMENT WHILE PROMOTING INDEPENDENCE AND INTEGRATION INTO THE LOCAL COMMUNITY.
