SUNCOAST PACE (PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY) - PACE IS A NURSING HOME DIVERSION PROGRAM THAT PROVIDES A FULL RANGE OF HOME AND HEALTH CARE SERVICES TO SENIORS 55 YEARS OF AGE OR OLDER. PACE HELPS PEOPLE MEET THEIR HEALTH CARE NEEDS IN THE COMMUNITY INSTEAD OF GOING TO A NURSING HOME OR OTHER CARE FACILITY. THE PROGRAM OFFERS PRIMARY AND SPECIALITY MEDICAL CARE, ADULT DAY CENTER SERVICES, HOSPITALIZATION AND EMERGENCY CARE, NUTRITIOUS MEALS, IN-HOME SUPPORT AND CARE, TRANSPORTATION SERVICES, PHARMACY SERVICES, AND PHYSICAL, OCCUPATIONAL AND SPEECH THERAPIES. IN ADDITION TO MEDICAL SERVICES AND THERAPIES, THERE ARE NUMEROUS ACTIVITIES AND OPPORTUNITIES TO SOCIALIZE. PACE'S GOAL IS TO KEEP THE PARTICIPANTS IT SERVES LIVING SAFELY IN THEIR OWN HOMES OR TOGETHER WITH THEIR FAMILIES AS LONG AS POSSIBLE WHILE MAINTAINING THEIR INDEPENDENCE AND QUALITY OF LIFE. AT SEPTEMBER 30, 2024, 338 PARTICIPANTS WERE BEING SERVED BY THIS PROGRAM.
