Since 2022, we have launched a new series of monthly community outreach webinars in Mandarin, some with simultaneous interpretation in Taiwanese, Cantonese, Vietnamese, English, and other Asian languages when resources were available. Topics have included ongoing COVID-19 updates and resources, as well as general medical, health, and wellness education and promotion. Over one hundred free educational webinars have been held to support the Asian American community, benefiting more than ten thousand attendees. We also continued the COVID-19 emotional support hotline to comfort individuals who were deeply impacted by the pandemic and experienced grief and loss. In addition, we maintained online support groups to assist cancer patients, as well as to support families and caregivers via telephone. In August 2022, the organization reinstated its medical and clinical care services and began accepting patients in October. In March 2023, the organization successfully passe d the accreditation process by the Centers for Medicare & Medicaid Services (CMS) and became a Medicare-certified hospice provider.The Organization remained in good standing and successfully passing the CHAP survey again on January 9, 2026 and continues to be accredited as a Medicare hospice provider through March 16, 2029. The program reflects the spirit and philosophy of care that emphasizes comfort and dignity for the dying, enabling them to remain independent for as long as possible and in familiar surroundings. Hospice care is centered on both the patient and their family or caregivers. Hospice personnel respect and respond to the unique differences in each familys lifestyle, values, and wishes. Utilizing an interdisciplinary team approachincluding an administrator, Director of Patient Care Services (DPCS), nurses, medical social workers, home health aides, volunteers, spiritual counselors, a bereavement coordinator, and other allied health professionals such as occupational, physical, and speech therapistsHeart of Hope Asian American Hospice Care provides hospice services wherever the patient resides. This includes coordination of transportation, ancillary services, and bereavement support. Services are equitably distributed to a medically and financially diverse group of patients in a cost-effective manner, ensuring alignment with the mission of Heart of Hope Asian American Hospice Care and supporting the organizations financial sustainability. Within its capacity, the organization also strives to provide charity care to patients who lack insurance coverage or have limited benefits.
