That's why Medicare is working with many doctors, hospitals and other health care providers to promote better, more coordinated health care. To allow this coordination, Hartford HealthCare Medical Group has created a team-based approach to medicine known as an accountable care organization. The Hartford HealthCare Accountable Care Organization, Inc. (HHC ACO) works to ensure that your providers are working together to help you get the right care at the right time in the right setting.
