Hospice services are provided through an interdisciplinary team approach, directed by the patient's own physician. Hospice services allow persons to face the reality of a life-threatening illness with hope and dignity, surrounded by family and friends. Physical, psychological, social and spiritual care are available from the medically directed team consisting of: physician/nursing care, home health aide, social work, pastoral care, volunteers and bereavement.

PRAIRIE HOME HEALTH DIVISIONThe main purpose of this division is to provide medical care and supportive services to recovering, disabled or chronically ill persons who need medical treatment or assistance with daily living. Through the home health division, clients receive quality skilled services in the comfort and security of their own homes. Services available include nursing care, home health aide and restorative therapy (physical, occupational, speech).
