PRAPARE is evidence-based, designed through stakeholder engagement, paired with an Implementation and Action Toolkit, and standardized across ICD-10, LOINC, and SNOMED. It is estimated just 20% of health outcomes are attributed to clinical care, while social drivers account for the remaining 80%.

This effort includes data collection and/or analysis of data associated with projects related to objectives of this effort. CHA and its partners develop content for technical assistance and training for health centers to deploy strategies for attaining the objectives of this effort (e.g., white papers, webinars, speaking engagements).
