COMMUNITY ALTERNATIVES PROGRAM (CAP) SERVICES ARE PROVIDED FROM THE WINDSOR AND KENANSVILLE AGENCY LOCATIONS. DURING THE 2024 FISCAL YEAR, HOME HEALTH SERVICES WERE PROVIDED TO 3,049 PATIENTS FOR A TOTAL OF 34,240 VISITS WHILE MAINTAINING A 3.5 QUALITY STAR RATING AT THE GREENVILLE, WINDSOR AND KENANSVILLE LOCATIONS AND A 3.0 AT THE WASHINGTON LOCATION.
