HOSPICE & COMMUNITY CARE PROVIDES COMPREHENSIVE MANAGEMENT OF PHYSICAL, EMOTIONAL, SOCIAL AND SPIRITUAL NEEDS OF PATIENTS. THE GOAL OF CARE IS TO ACHIEVE THE BEST POSSIBLE QUALITY OF LIFE THROUGH RELIEF OF SUFFERING, CONTROL OF SYMPTOMS AND RESTORATION OF FUNCTION CAPACITY. THROUGH AN INTERDISCIPLINARY, MEDICALLY DIRECTED TEAM OF PROFESSIONALS AND VOLUNTEERS, PHYSICAL, SOCIAL, EMOTIONAL AND SPIRITUAL SUPPORT IS PROVIDED TO PATIENTS AND THEIR FAMILIES. THE CARE IS EXPRESSLY TAILORED TO EACH PATIENT'S NEEDS AND WISHES AND IS FOCUSED ON TREATING THE PERSON, NOT THE DISEASE. TEAMS INCLUDE A PHYSICIAN, REGISTERED NURSES, SOCIAL WORKERS, CERTIFIED NURSING ASSISTANTS, GRIEF COUNSELORS, CLERGY AND VOLUNTEERS. HOSPICE & COMMUNITY CARE ALSO PROVIDES A BROAD RANGE OF (CONTINUED ON SCHEDULE O)PROGRAMS, EDUCATIONAL SEMINARS AND COUNSELING OPPORTUNITIES TO HELP HOSPICE FAMILIES AND THE COMMUNITY COPE WITH GRIEF AND LOSS. IN ADDITION TO HOME CARE SERVICES, HOSPICE & COMMUNITY CARE OPERATES A 16 BED HOSPICE HOUSE CALLED THE WAYNE T. PATRICK HOSPICE HOUSE. THE PATRICK HOUSE IS USED TO PROVIDE HOSPICE PATIENTS WITH EXPERT CARE FOR PAIN CONTROL AND SYMPTOM MANAGEMENT, TO OFFER CAREGIVERS A PLACE OF RESPITE, AND TO PROVIDE A PLACE FOR PATIENTS WHO MAY NEED THE SAFETY AND SECURITY OF SHORT TERM RESIDENCE.HOSPICE & COMMUNITY CARE PROVIDES ITS SERVICES TO PATIENTS REGARDLESS OF THEIR ABILITY TO PAY FOR THOSE SERVICES. THE ORGANIZATION ESTIMATES THAT IT PROVIDED SERVICES AND SUPPLIES UNDER ITS CHARITY CARE POLICY OF APPROXIMATELY $40,000 DURING 2023.
