BayCare Home Care, Inc is a multi-service home care provider serving west central Florida that includes service lines including: * Certified and Licensed Home Health agencies that provide skilled and supportive services in the home including skilled nursing, rehabilitation therapies, medical social work, home health aide and other services. These agencies are licensed to serve eligible patients living in 13 counties of west central Florida. * Durable Medical Equipment (DME) / Home Medical Equipment provider that delivers and supports equipment including, but not limited to, home medical equipment wound care devices, delivery of oxygen in the home, home sleep medicine equipment and supplies, seating and mobility devices, orthotic and prosthetic devices and supplies, and various other medical supplies * Home Infusion Pharmacy and Specialty Pharmacy Services including medication, supplies and services. * Service line that provides Respiratory Therapy support and clinical expertise to the region's nursing homes and other post acute programs. The various programs have a consistent record of safety compliance and clinical accomplishment including ongoing accreditation by the Joint Commission on HealthCare Accreditation as a home care multi-service entity, a solid history of state regulatory compliance and average Centers for Medicare & Medicaid Services (CMS) Quality Stars rating of 4.0 across home health. Pharmacy and DME compliance are also stellar with CMS, Agency for Health Care Administration (Florida's state regulatory agency) and the Florida Board of Pharmacy.
