ADULT DAY HEALTH (ADH) OFFERS A MEDICAL MODEL OF ADULT-DAY PROGRAMMING THAT PROVIDES SPECIALTY CARE TO PEOPLE LIVING WITH CHRONIC CONDITIONS AND LONG-TERM DISABILITIES, INCLUDING SERVICES FOR INDIVIDUALS WITH ALZHEIMER'S DISEASE AND RELATED TYPES OF DEMENTIA.
