IS THE SOLE SHAREHOLDER OF EPIC MEDICAL. EFFECTIVE APRIL 1, 2016, THE NEW YORK STATE OFFICE FOR PEOPLE WITH DEVELOPMENTAL DISABILITIES ("OPWDD") ELIMINATED THE PROVISION OF OFFSITE ARTICLE 16 SERVICES TO INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES AND INTRODUCED A NEW MEDICAID STATE PLAN SERVICE KNOWN AS INDIVIDUAL PRACTITIONER SERVICES FOR INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES ("IPSIDD"). THIS DEVELOPMENT LED TO THE FORMATION OF EPIC MEDICAL, TO ALLOW PROVISION OF THESE SERVICES. IPSIDD SERVICES ARE LIMITED TO PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY, SOCIAL WORK, AND PSYCHOLOGICAL SERVICES THAT MAY BE PROVIDED TO INDIVIDUALS IN SERVICE ARRANGEMENTS SUBJECT TO PRIOR AUTHORIZATION FROM OPWDD.
