HEALTHCOMM EXPANDED ITS FOCUS ON FOOD SECURITY IN EATON COUNTY WITH COLLABORATIVE PARTNERS THAT INCLUDED THE REGIONAL AND LOCAL FOOD BANKS, THE LOCAL HOSPITAL AND HEALTH DEPARTMENT. THE EXPANSION OF THIS PROGRAM CONSISTED OF AN ADDITIONAL SET OF SMART, REFRIGERATOR LOCKERS IN A NEW COMMUNITY, AND THE DEVELOPMENT OF AN EATON COUNTY FOOD LOCKER ADVISORY COMMITTEE. EATON COMMUNITY HEALTH (ECH) PARTNERED WITH CHARLOTTE PUBLIC SCHOOLS AS PART OF OUR HEALTHCOMM INITIATIVE. THE CHARLOTTE PUBLIC SCHOOLS COLLECT COMMUNITY DONATIONS TO SUPPORT CANCER PATIENTS AT LOCAL SPORTING EVENTS. THOSE FUNDS ARE PROVIDED TO ECH TO PURCHASE AND DELIVER GAS CARDS TO HEALTHCARE PROVIDERS THROUGHOUT THE COUNTY TO BE GIVEN TO ANY EATON COUNTY RESIDENT UNDERGOING TREATMENT FOR CANCER, PROVIDING SUPPORT AND ACCESS TO CARE.

SUCCESSFUL EXPANSION OF THE HSCC INITIATIVE (CALLED HUMANSERV), IN COLLABORATION WITH THE EATON COUNTY HUMAN SERVICES COLLABORATIVE COUNCIL (HSCC). DURING THIS FISCAL YEAR, EATON COMMUNITY HEALTH SERVED AS A CATALYST AND CONVENER FOR THE COLLABORATIVE, WHICH NOW HAS THREE ACTION TEAMS, AND COMPLETED A COMPREHENSIVE SURVEY OF FRONT-LINE WORKERS AND A SECOND LARGE SCALE RESIDENT SURVEY RELATED TO ACCESS TO RESOURCES IN THE COUNTY. SUCCESSFULLY EXPANDED STREETHEART, EATON COMMUNITY HEALTH'S RESIDENT ENGAGEMENT PROGRAM, DESIGNED TO IMPROVE THE HEALTH AND VITALITY WITHIN THE COUNTY, ONE NEIGHBORHOOD AT A TIME. THE PILOT NEIGHBORHOOD WORK HAS EXPANDED FOR THE 300 HOUSEHOLDS. THE STREETHEART TEAM NOW INCLUDES A NEIGHBOR LEADERSHIP PROGRAM THAT INCLUDES SIX BLOCK LEADERS IN A GROWING RESIDENT ADVISORY GROUP. THE SECOND NEIGHBORHOOD WELL-BEING ASSESSMENT WAS COMPLETED. STREETHEART ALSO FACILITATED AND FUNDED A SUCCESSFUL MINI GRANT IN PARTNERSHIP WITH UNITED METHODIST CHURCH OF CHARLOTTE CALL " ROCK THE CROCK" IN WHICH 9 FAMILIES WERE PROVIDED ONE MEAL A WEEK TO COOK TOGETHER AND CONNECT WITH NEIGHBORS.
