Throughout FY2025 Heart Mind Haven offered unique individualized and customized programs centered in human connection that specifically addressed the unique needs of the queer community and provided confidence and compassionate spirit to trans-gender young adults. Heart Mind Haven was very successful in advancing health equity for 68 unique individuals, and closing gaps in care and building systems recovery that address the unmet needs of our LGBTQIA community. Heart Mind Haven successfully helped address the underlying causes of suicide, substance use and addiction for this most vulnerable population that is so prevalent and pervasive. 1. Over 97% of our residents at HMH will stay engaged with treatment, counseling, mental health care and sobriety; this outcome has exceeded our expectations and current standards for sober living organizations.2. 95% of residents at HMH gained employment, several completed GEDs, and/or entered secondary education programs. Also 5 individuals have entered career training programs in state funded programs.3. Heart Mind Haven is currently receiving an unprecedented number of applications for recovery housing from dozens of treatment centers and other programs throughout the state and national. Heart Mind Havens objective of ending isolation and providing support to the LGBTQIA community that are at higher risk of suicide is currently decreasing residents visits to hospital emergency rooms and detox centers. This evidenced by only 2 incidents of this out of 58 individuals approximately 78%.Heart Mind Haven offers unique and customized programs centered in human connection that specifically address the unique needs of the queer community and provides confidence and compassionate spirit to trans-gender young adults. Heart Mind Haven advances health equity in multiple areas for these individuals by closing gaps in care and building systems recovery that address the unmet needs of our LGBTQIA community members to address the underlying causes of suicide, substance use and addiction for this most vulnerable population.
