THROUGH OUR FOOD AND EXTREME WEATHER PROGRAMS, WE ALSO SERVED 300-400 INDIVIDUALS WITH SURVIVAL GEAR, FOOD AND CARING HUMAN CONTACT. OF THOSE WHO WE ASSISTED TO FIND HOUSING, SIX WERE PROVIDED A PERMANENT HOME WITH CASE MANAGEMENT SUPPORT. OUR SUCCESS COMES FROM INDIVIDUALIZED ATTENTION AS WELL AS COLLABORATION WITH OTHER AGENCIES, PARTICULARLY HOMEFIRST. AND PG&E. OUR OUTREACH WORKERS WERE ON THE STREET CONNECTING CLIENTS WITH SERVICES AND WORKING TO SUPPORT THEIR PERSONAL DIGNITY AS THEY MADE A BETTER LIFE.
