SFV HEALTH CENTER, INC. DBA: ELEVATE HEALTH CENTER (EHC) PLAYS A MAJOR ROLE AS PART OF THE "SAFETY NET" THROUGHOUT THE SAN FERNANDO AND ANTELOPE VALLEYS OF LA COUNTY. EHC OFFERS GENERAL PRIMARY MEDICAL CARE, SCREENINGS, VOLUNTARY FAMILY PLANNING, GYNECOLOGICAL CARE, PRENATAL CARE, POSTPARTUM CARE, IMMUNIZATIONS, WELL CHILD SERVICES, PHARMACEUTICAL SERVICES, DIAGNOSTIC LABORATORY, AND ENABLING SERVICES SUCH AS CASE MANAGEMENT, ELIGIBILITY ASSISTANCE, HEALTH EDUCATION, OUTREACH, TRANSPORTATION, AND TRANSLATION SERVICES TO INDIVIDUALS AND FAMILIES IN A PATIENT-CENTERED, INTEGRATED MODEL. ADDITIONAL SERVICES ARE AVAILABLE BY REFERRAL INCLUDING DIAGNOSTIC RADIOLOGY, MAMMOGRAMS, LABOR AND DELIVERY, AND PREVENTIVE DENTAL CARE. SERVICES ARE CULTURALLY AND LINGUISTICALLY APPROPRIATE, HIGH-QUALITY, AND COMPREHENSIVE. EHC CONTINUES TO ADDRESS THE NEED FOR PRIMARY CARE SERVICES IN THE COMMUNITY AND TARGET POPULATION BY CONTINUING SERVICES WITH THE GOAL OF INCREASING HEALTHCARE ACCESS, IMPROVING HEALTH OUTCOMES, AND REDUCING HEALTH DISPARITIES. DURING THE YEAR, THE ORGANIZATION PROVIDED SERVICES TO 3,557 DISTRICT PATIENTS THROUGH 15,720 DISTRICT VISITS.
