PLANT A SEED PROVIDES DEVELOPMENTALLY DELAYED CLIENTS WITH A SAFE, NURTURING, AND THERAPEUTIC HOST HOME PROGRAM. THE PROGRAM OFFERS NEED-BASED SUPPORTS TO MEET EACH INDIVIDUAL'S NEEDS AND TO WORK HAND IN HAND WITH THEM AND THEIR FAMILIES TO PROVIDE SERVICES, RESOURCES AND SUPPORT TO BETTER THE INDIVIDUALS SERVED QUALITY OF LIFE. WHILE THE PRIMARY FOCUS OF THE PROGRAM IS RESIDENTIAL SERVICES, PLANT A SEED STAFF ALSO ASSISTS IN THE COORDINATION OF OUTSIDE SERVICES, MEDICAL NEEDS, AND COMMUNITY INTEGRATION. HOST HOMES ARE PROVIDED BY EXTENSIVELY SCREENED AND TRAINED FAMILIES WHO WELCOME AND INTEGRATE PERSONS WITH DEVELOPMENTAL DISABILITIES INTO THEIR FAMILY AND HOME. THE PROGRAM PROVIDES 24-HOUR SUPPORT TO CLIENTS. INDIVIDUALS RESIDING IN PLANT A SEED HOST HOMES RECEIVE A HIGH LEVEL OF SUPPORT IN AREAS SUCH AS, BUT NOT LIMITED TO, DAILY LIVING SKILLS, SHOPPING, HYGIENE SKILLS, COMMUNITY INTEGRATION, TRANSPORTATION AND MEDICATION MANAGEMENT. HOST HOMES ARE REGULARLY MONITORED AND EXTENSIVELY TRAINED. HIGH STANDARDS ARE IMPLEMENTED AND UPHELD WHILE FOCUSING ON PERSON-CENTERED PREFERENCES.
