SERVING CAPE COD AND THE ISLANDS, THE LILY HOUSE PROVIDES PERSONALIZED, SKILLED, AND COMPASSIONATE END-OF-LIFE CARE. EACH YEAR, WE CARE FOR APPROXIMATELY 25-30 INDIVIDUAL RESIDENTS DURING THEIR FINAL WEEKS AND DAYS OF LIFE, WHILE ALSO PROVIDING SUPPORT FOR OVER 200 FRIENDS AND FAMILY MEMBERS OF RESIDENTS. IN ADDITION TO PROFESSIONALLY-TRAINED RESIDENT CARE PROVIDERS, A DEDICATED TEAM OF TRAINED COMMUNITY VOLUNTEERS HELP DELIVER CARE AND COMPANIONSHIP. OUR CARE IS HOLISTIC, WE PROVIDE PHYSICAL, EMOTIONAL, SOCIAL, SPIRITUAL, EDUCATIONAL, AND LOGISTICAL SUPPORT BEFORE, DURING, AND AFTER-DEATH. THE LILY HOUSE IS A SOCIAL MODEL COMMUNITY HOSPICE HOME AND DELIVERS CARE IN PARTNERSHIP WITH THE RESIDENTS MEDICAL HOSPICE TEAM. THE LILY HOUSE ALSO SERVES AS A COMMUNITY RESOURCE FOR END-OF-LIFE EDUCATION AND RESOURCES. WE PROVIDE FREE WORKSHOPS ON TOPICS SUCH AS GREEN BURIALS, HOME FUNERALS, AND ADVANCE DIRECTIVES, AND OFFER CONVERSATION EVENTS, SUCH AS DEATH CAFES. WE ALSO PARTNER WITH OTHER MISSION-ALIGNED ORGANIZATIONS TO PROVIDE A COMMUNITY-CENTERED CONTINUUM OF CARE. THE LILY HOUSE IS A MEMBER HOME OF THE OMEGA HOME NETWORK, A NATION-WIDE NETWORK OF SOCIAL MODEL HOSPICE HOMES WHOSE MISSION IS TO SUPPORT THE HIGHEST QUALITY END OF LIFE EXPERIENCE POSSIBLE, FOR THE DYING PERSON AND THEIR LOVED ONES. WE ARE ALSO PART OF A NATIONAL GRASSROOTS MOVEMENT CREATING A CULTURAL SEA CHANGE IN HOW WE THINK ABOUT, TALK ABOUT, AND PREPARE FOR END-OF-LIFE.
