Immaculate Heart of Mary Family Health Center is a non-profit charitable primary care clinic in Chandler, AZ. Our mission is to provide excellent primary care services to people of all socioeconomic backgrounds in a way that is wholly consistent with the Ethical and Religious Directives of the Catholic Church ERDs. We are committed to providing medical care to everyone, including the poor and underserved in our community, in a way that reflects the love and mercy that God has for all of our patients. We provide care to the poor and underserved by providing a sliding fee scale payment system that allows people living under 200 percent of the Federal Poverty Level to receive greatly discounted appointment fees, procedure fees, as well as discounted in-office testing and medications. We also participate in the Vaccine for Children VFC program which allows us to distribute free immunizations to the underserved pediatric patients in our state. We do not deny care to anyone because of inability to pay, we do not limit the number of patients who participate in our sliding fee scale, nor the number of medicare or medicaid patients that we care for. In 2023 we provided 45 reduced-cost or free appointments to the underserved in our community, 142 medicaid visits, 461 medicare visits and added 8 patients to our charity care program. We set no limit or cap on charitable visits or charity-care patients. We also participated in the Vaccines for Children program and were able to provide over 50 immunizations for free to deserving patients.
