Alivia Supportive Care provides specialized, person-centered, coordinated care to people with late-stage, serious life-limiting illness. Our program moves the focus of care out of the hospital and into the home and community. Our dedicated Palliative Care Team provides patients with the tools to manage their illness and improve quality of life. Through regular home visits, our APRNs help create a personalized care plan unique to the individual and based on their needs. Patients and families benefit from our program by receiving practical and emotional support, guidance in planning and making decisions about future health care needs, peace of mind navigating complex health care systems, and relief from pain and challenging symptoms. Serving over 50 zip codes in Duval, Clay, Nassau, St. Johns, and Baker counties, this year we are projected to treat over 950 patients. To help our patients avoid unnecessary hospitalizations and visits to the emergency room, we coordinate with primary care and specialty physicians, connect patients to community resources, and provide education about serious illness and end of life planning. In addition to APRN home visits, we offer a 24-hour call center staffed by RNs, RN and Social Worker telephonic and home visits, and a well-trained operations team. Although most of these services are not covered by insurance plans, we are committed to our program and believe these services provide value to our patients and their loved ones.
